This full-time position offers a competitive annual salary ranging from USD 87,000 to 105,000, depending on experience and qualifications. The role is designed for a remote counselor who will provide outpatient services. You will utilize telehealth technologies to conduct client assessments and therapy sessions, thereby accommodating various client needs and schedules.

Key Responsibilities\n

• Collaborate with primary care providers to identify, assess, and manage patients with mental health conditions and psychosocial issues, working as an integral member of a multidisciplinary team.

• Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations, and formulate tailored treatment plans based on individual patient needs.
\n
• Provide brief, focused interventions for clients requiring immediate mental health support, utilizing evidence-based therapeutic techniques.
\n
• Guide primary care providers in determining which patients would benefit from continued care within the primary setting versus those needing referrals to specialized mental health facilities.
\n
• Continuously monitor patient progress and initiate follow-ups to assess the effectiveness of treatment approaches, making necessary adjustments as indicated.
\n
• Develop, implement, and monitor relapse prevention strategies to ensure ongoing patient stability and support.
\n
• Identify clients at risk of worsening conditions; devise and implement prevention plans to mitigate potential deterioration of their mental health.
\n
• Document all interactions thoroughly in the client's treatment chart and ensure that all progress notes are compliant with agency and Federally Qualified Health Center (FQHC) standards.

• Engage clients in self-management strategies and provide educational resources to empower them in their recovery journey.
\n
• Consult with clinic management and team members on the effectiveness of behavioral health services and participate in outcome evaluations to enhance program effectiveness.
\n
• Support patients in adhering to medical treatment plans established by primary care providers, helping them manage any medication side effects with effective coping strategies.

• Adhere to the established Code of Ethical Conduct for Clinical Professionals, maintaining professionalism and integrity in all interactions.

You will primarily serve a diverse clientele, including individuals facing various mental health challenges such as anxiety, depression, trauma, and more. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling is required.

• Possession of a current LPC (Licensed Professional Counselor) License in the state of Pennsylvania.

• Strong knowledge of mental health assessment practices, treatment planning, and developmental psychopathology, along with familiarity with DSM-IV diagnoses and behavioral therapy techniques.

• Ability to communicate effectively, with bilingual proficiency in English and Spanish preferred, although not mandatory.

• Demonstrated mental and physical health to fulfill the demands of the counselor role.
\n
• An understanding of the social and personal implications of mental health issues on individuals and their families.

• Extensive knowledge of community resources and support systems to aid clients outside of therapy.
