Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and making informed recommendations for treatment plans to primary care providers.

• Provide brief, targeted interventions for clients requiring immediate mental health support, responding sensitively to their unique needs.

• Actively participate in treatment planning by advising the primary care team on mental health matters and effectively triaging patients to specialty mental health services when necessary.

• Engage in regular follow-up with patients to monitor progress, identify potential changes in treatment approaches, and adjust plans to ensure optimal care.

• Create relapse prevention strategies and support patients in maintaining stable functioning through individualized support and guidance.
\n
• Identify at-risk patients and develop proactive plans to prevent deterioration of their mental and emotional health.

• Document all patient interactions, progress, and diagnostic information meticulously to ensure compliance with healthcare regulations and enhance care delivery.

• Collaborate with primary care providers to formulate comprehensive treatment plans, ensuring continuous communication regarding patient needs and strategies.

• Equip clients with self-management skills and education to empower them in their recovery journeys.

• Provide behavioral health consultations to clinic management and other staff, contributing to outcome assessment and program evaluation.

• Assist patients in complying with medical treatments recommended by primary care providers, including addressing medication side effects and lifestyle changes.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field is required.
