Work Environment

This full-time position offers an onsite working environment, where you will deliver care directly in patients' homes throughout the Princeton area. Working hours are primarily Monday to Friday, from 9 AM to 5 PM, with occasional flexibility required to accommodate patient needs. The role allows therapists to work independently while being supported by an interdisciplinary team of healthcare professionals, ensuring a holistic approach to patient care.

Key Responsibilities

• Conduct thorough in-home physical therapy evaluations to assess each patient's conditions and needs.
• Develop and implement individualized treatment plans that address specific goals related to post-surgical recovery, orthopedic rehabilitation, neurological conditions, and overall functional improvement.
• Administer a range of therapeutic interventions, including targeted exercises, gait training, balancing activities, and education on strategies for enhancing mobility.
• Provide guidance to patients and caregivers on home safety measures, fall prevention techniques, and wellness strategies that promote long-term health.
• Monitor patient progress meticulously and adapt treatment plans as necessary to align with evolving functional goals.
• Maintain accurate documentation for each visit in compliance with Medicare and agency standards, ensuring timely submission of required reports.
• Coordinate and communicate effectively with an interdisciplinary team, including nursing staff, occupational therapists, speech-language pathologists, and social workers, to ensure comprehensive patient care.

Your expertise will directly impact a diverse population, including elderly patients, individuals recovering from surgery, and those dealing with chronic conditions. You will provide essential support for their rehabilitation journey, making a significant difference in their everyday lives by enhancing their mobility and independence.

Qualifications

• A valid New Jersey Physical Therapist license is required.
• A Bachelor's degree in Physical Therapy from an accredited program (DPT or MPT).
• Possession of a valid driver's license along with reliable transportation and auto insurance.
• Current CPR/BLS certification.
• Exceptional clinical judgment and time-management abilities are essential for success in this role.
• The ability to work independently while effectively managing a caseload of patients.

Preferred Experience

• A minimum of 2 years of clinical experience in home health, outpatient, or hospital rehabilitation settings.
• Familiarity with electronic medical records (EMR) systems such as HomeCare HomeBase, Kinnser, or MatrixCare.
• Knowledge of Medicare documentation requirements and OASIS is preferred; however, training will be provided for those who need it.

Compensation and Benefits
