You will work in a climate-controlled office setting located within our Hospice Center in San Angelo, Texas, where you will enjoy daily interactions with fellow healthcare providers and department staff. Key Responsibilities\n

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Assessing and diagnosing clients when necessary, utilizing tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to evaluate each patient's mental health and develop individualized care plans.

• Employing continuous assessment strategies to track patient progress and providing appropriate interventions to support their mental health journey.

• Maintaining accurate and timely documentation of treatment records, reports, and clinical information using current technology, in accordance with regulatory requirements.

• Collaborating with patients and their families to establish comprehensive treatment plans tailored to promote their emotional and psychological well-being.

• Coordinating referrals and acting as a liaison between patients and treatment providers, ensuring that each individual receives the necessary care and support.

Qualifications\n
