Key Responsibilities

• Conduct thorough health assessments and screenings, collaborating with patients to create individualized care plans that address both physical and mental health issues.
• Diagnose and manage acute and chronic medical conditions, with a special emphasis on mental health care, preventive services, and health promotion initiatives.
• Engage in meaningful collaborations with local community organizations and stakeholders to devise and implement health education programs that enhance health literacy.
• Deliver culturally sensitive care that advocates for the rights of all patients, ensuring they have access to necessary healthcare services.
• Coordinate referrals and follow-up care, actively ensuring continuity of care and effective management of patients’ healthcare journeys.

Patient Population / Client Focus

You will primarily serve a diverse patient population that includes individuals and families experiencing mental health challenges and other chronic conditions, particularly those from underserved backgrounds. Your role is vital in taking a comprehensive approach to care that prioritizes not only medical management but also emotional and social support.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in Nursing from an accredited program.
• Solid understanding of mental health principles and practices as evidenced by at least three years of clinical experience.
• Exceptional interpersonal and communication skills to effectively connect with patients from diverse backgrounds.
• A strong commitment to social justice, healthcare equity, and advocacy for vulnerable communities.

Preferred Experience

Compensation and Benefits
