Key Responsibilities\n

• Initiate discharge planning from the moment of patient admission, taking proactive steps to ensure patients have an actionable exit strategy upon their return to the community.

• Conduct discharge sessions that confirm patients' understanding of their coping strategies, transition plans, and available resources, ensuring a smooth transition post-care.

• Coordinate communication with vital referral sources and outpatient providers to uphold continuity of care, checking in at various points: admission, mid-treatment, and discharge.

• Facilitate patient transfers to higher levels of care when necessary, advocating for the best possible outcomes for our patients.

• Actively participate in clinical outreach initiatives and provide program tours to engage with the community and promote our services.

• Manage crises that may arise during therapy sessions or within the milieu effectively, utilizing de-escalation techniques and therapeutic interventions.

• Collaborate with milieu staff, providing guidance and support in their interactions with patients to create a therapeutic environment.

• Contribute to the clinical team’s success by completing all assigned tasks and assisting in clinical support during colleagues’ absences.

• Assess patients for suicidality and potential harm, ensuring immediate intervention and safety measures are in place.

• Work closely with the Utilization Review and Business Departments regarding authorizations and payment processes to ensure efficient service delivery.
