Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) for Home Health in Lake Zurich, Illinois. This full‑time position supports patients in Palatine, Huntley, Crystal Lake, and surrounding areas. Responsibilities Lead and collaborate to manage and deliver patient care plans. Provide nursing care at patient homes and ensure continuity of care. Maintain accurate and timely charting to meet critical documentation deadlines. Self‑manage time and schedule to optimize patient visits. Utilize tablets, mobile phones, and EMR software for clinical documentation. Travel approximate 20% of the day to and from patient locations. Qualifications Current Registered Nurse license in the state where you will work. Graduate of an approved school of practical nursing. Minimum one year of RN experience with the clinical competence to deliver quality patient care. Current driver’s license and ability to drive to patient locations. Commitment to consistently meet critical charting deadlines. Experience with tablets, mobile phones, and EMR software.
