Work Environment\n

This position is primarily remote but requires flexibility to travel to various CHDC sites for meetings, off-site retreats, conferences, and outreach functions. Key Responsibilities\n

Your day-to-day responsibilities will encompass a wide range of clinical and administrative duties aimed at promoting integrated health care. Specific tasks include:

• Assisting mental health consumers in navigating both physical and behavioral healthcare systems to ensure seamless access to comprehensive services.

• Educating patients about available resources and services to support their participation in disease prevention, wellness activities, and overall healthy lifestyle choices.
\n
• Actively participating in multi-disciplinary treatment team meetings to collaborate on patient care strategies.
\n
• Providing timely reporting of all crisis situations to the Medical Director, with immediate notification of psychiatric and medical emergencies to the Chief Medical Officer (CMO).
\n
• Documenting detailed clinical notes for each patient in their clinical records, ensuring all information is accurate and up-to-date.
\n
• Engaging in the Quality Improvement program through active participation in assigned meetings and training sessions.
\n
• Maintaining compliance with all internal policies and procedures, with particular attention to confidentiality and ethical standards in integrated primary care behavioral health settings.

• Utilizing evidence-based screening tools to assess patient health needs and documenting findings appropriately.
\n
• Collaborating with the Center of Excellence Team to support patients dealing with Opioid Use Disorder, ensuring they receive comprehensive care tailored to their unique needs.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in a hospice care setting, many of whom may be experiencing complex emotional and psychological challenges due to end-of-life issues. Your expertise will not only address their immediate mental health needs but also provide support to families, ensuring they are equipped to manage the emotional aspects of this critical stage in their lives. Your compassionate approach will be pivotal in helping patients achieve peace and dignity.

Qualifications\n

The successful candidate will possess the following qualifications:

• A minimum of a master’s degree in social work, with licensure as a Licensed Clinical Social Worker (LCSW) required.

• At least 2 years of relevant experience in the social work field, preferably within a healthcare setting such as primary or specialty care.

• Demonstrated ability to work effectively in a collaborative environment with healthcare professionals, social service providers, and clients.

• Willingness to engage in travel to CHDC sites as needed for meetings and outreach efforts.

Preferred Experience\n

While not mandatory, candidates with experience in hospice or palliative care, as well as those familiar with integrated care models, will be given preference. Experience with managing cases involving substance use disorders, particularly Opioid Use Disorder, is also advantageous.

Compensation and Benefits\n

CHDC offers a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience and qualifications.
