Key Responsibilities\n

• Conduct thorough assessments to evaluate patient needs and develop appropriate treatment plans tailored to each individual's circumstances.

• Maintain ongoing communication with supervisors, families, and other stakeholders to provide updates on patient progress, ensuring everyone involved is informed and engaged.

• Create tentative schedules for client appointments and home visits, ensuring that all appointments are effectively managed and documented.

• Coordinate and execute home visits as needed to fulfill the requirements of in-home treatment and support, carrying out thorough chart reviews and staff interviews.

• Implement and monitor the effectiveness of personalized treatment plans, making adjustments as necessary based on patient feedback and clinical assessments.

• Participate actively in Interdisciplinary Team Meetings to collaborate with other mental health professionals and contribute insights for integrated care.

• Document session notes and complete monthly and quarterly progress reports that accurately reflect client development and therapeutic milestones.

• Organize and facilitate training sessions for support staff to ensure they are knowledgeable about program plans and maintained documentation standards.

• Engage in follow-up interactions, including interviews and home visits, to provide continued support and ensure sustained progress for each client.

• Serve as a liaison with the broader mental health community to foster partnerships and promote organizational development goals.
