All Jobs > Deputy Director of Community Partnerships and Resource Navigation Deputy Director of Community Partnerships and Resource Navigation Boston • Practice Transformation The Deputy Director of Community Partnerships & Resource Navigation provides strategic and operational leadership for DotHouse Health's community resource and patient navigation programs. The position oversees initiatives that address health-related social needs (HRSN), strengthen community partnerships, and improve patients' access to clinical and community-based services. The Deputy Director is responsible for developing, implementing, and evaluating patient navigation models across multiple clinical programs, including community resource navigation, perinatal navigation, family navigation, and other specialized navigation services. Working in partnership with clinical leaders, the Deputy Director ensures navigation programs are effectively integrated into care delivery, supported by strong workflows, community partnerships, data-driven improvement, and sustainable funding. Program Leadership & Operations Provide strategic leadership and operational oversight for DotHouse Health's community resource and patient navigation programs. Supervise the Community Resource Team Manager, Perinatal Patient Navigator, Complex Patient Navigator, and other specialized navigation staff and programs as assigned. Develop staffing models, workflows, quality standards, productivity expectations, and cross-training initiatives. Recruit, mentor, coach, and evaluate staff and supervisors. Provide consultation and support for complex behavioral health, substance use, medical, and social navigation needs. Clinical Collaboration & Care Integration Partner with clinical leaders to develop, implement, and continuously improve patient navigation models and workflows. Coordinate multidisciplinary case review and ensure navigation services are integrated across primary care, behavioral health, reproductive health, substance use disorder, infectious disease, and other clinical programs. Collaborate with clinical leaders to establish referral processes, documentation standards, and quality improvement initiatives. Community Partnerships & Program Development Develop and maintain partnerships with community organizations and external agencies to strengthen resource availability and referral pathways. Lead organizational initiatives related to health-related social needs (HRSN), community resource navigation, and social care integration. Lead implementation of grant-funded navigation initiatives and participate in grant development, reporting, and sustainability planning. Evaluate and implement technology solutions that support community resource referrals and navigation. Data, Quality & Organizational Improvement Develop systems to monitor program performance, patient outcomes, and community partnership effectiveness. Oversee program reporting for grants, internal monitoring, and organizational leadership. Use data to identify opportunities for quality improvement, workflow redesign, and health equity initiatives. Collaborate with Quality, Public Health programming, Clinical Informatics, and other departments to support value-based care and population health strategies. Other Represent DotHouse Health on external committees, coalitions, and community partnerships as appropriate. Perform other duties as assigned. Qualifications: Requirements Master's degree in Social Work, Public Health, Public Administration, Nursing, Human Services, or related field. Minimum three years of progressively responsible experience leading patient navigation, case management, community resource, population health, community health, public health, or related programs. Minimum two years of supervisory or leadership experience. Experience developing community partnerships and collaborative initiatives. Experience managing grant-funded programs preferred. Experience in Federally Qualified Health Centers, community health centers, accountable care organizations, or value-based healthcare preferred. Preferred: Demonstrated commitment to health equity and reducing barriers to care. Strong knowledge of health-related social needs, social determinants of health, and community resource systems. Excellent leadership, coaching, and staff development skills. Strong project management and organizational skills. Experience designing workflows and leading organizational change. Experience analyzing program data and using findings to drive improvement. Ability to collaborate effectively with clinical, operational, and community stakeholders. Excellent written and verbal communication skills. Experience with Epic and community resource referral platforms preferred. Strong working knowledge of Microsoft Office Suite.
