This remote position allows you to engage directly with patients facing complex psychiatric and medical challenges. Work Environment\n

This full-time position is primarily remote, allowing you flexibility while working in a community health clinic setting. Our team operates Monday through Friday, from 8:00 AM to 5:00 PM, with occasional evening shifts necessitated by patient needs. Key Responsibilities\n

• Conduct comprehensive mental health assessments to evaluate the interplay between patients' psychiatric conditions and their medical ailments.

• Develop and implement integrated treatment strategies that cater to psychiatric and medical necessities, ensuring an individualized approach for each patient.

• Facilitate individual and group therapy sessions employing evidence-based methods, such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), tailored to diverse patient needs.

• Collaborate with medical practitioners within our interdisciplinary team to ensure cohesive treatment plans and promote optimal health outcomes.

• Provide crisis intervention and support to patients experiencing mental health emergencies, enabling timely and effective assistance.

• Empower patients through education about the relationship between mental health and physical health, encouraging self-management of their conditions.

• Maintain accurate, confidential records that comply with all legal and ethical regulations to ensure patient safety and confidentiality.

• Engage actively in team meetings, case conferences, and interdisciplinary care coordination discussions, enhancing the quality of patient care.

Patient Population / Client Focus\n

Your primary focus will be on patients grappling with dual challenges of mental health disorders and chronic medical conditions, including but not limited to chronic pain, autoimmune disorders, and substance use disorders. Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LMFT, LPC, or LCSW in the state of Georgia.

• Proven proficiency in managing the mental health requirements of patients with co-occurring conditions.

• Experience with integrated care models and working collaboratively with medical professionals.

Preferred Experience\n

While a minimum of one year of relevant experience is required, candidates with additional experience in integrated healthcare settings, particularly with patients facing both mental and physical health challenges, will be given preference. A familiarity with continuity of care models and crisis intervention protocols will also be advantageous.

Compensation and Benefits\n
