Key Responsibilities\n

• Conduct thorough biopsychosocial assessments to identify patients' mental health, substance abuse, and medical needs, ensuring that care is culturally and age-appropriate.

• Deliver behavioral health services, including individual and group counseling sessions, skills-building, intake assessments, diagnosis, treatment planning, and crisis intervention tailored to patients' unique circumstances and history.

• Utilize electronic health record systems to document mental health evaluations, progress notes, and counseling interactions efficiently and accurately.

• Provide liaison services and social work support, connecting patients with local community resources, including medication assistance programs and support groups.

• Collaborate with Quality Improvement teams to enhance care delivery processes, ensuring data is collected to monitor the efficacy of implemented interventions.

Qualifications\n

• A valid and active state license as a Licensed Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n
