Key Responsibilities\n

• Collaborate with multidisciplinary healthcare teams to develop personalized treatment plans tailored to each client’s needs, ensuring all discharge plans are completed in accordance with psychosocial assessments and approved interventions.

• Establish and maintain professional relationships with community resource leaders, advocating for our organization during meetings and collaborative committees focused on addressing patients’ social needs.

• Provide one-on-one counseling and crisis intervention for patients, employing assessment techniques to identify emotional and behavioral challenges and tailor appropriate therapeutic approaches.

• Maintain open lines of communication with physicians and other healthcare professionals to report patient progress, address concerns, and troubleshoot any problems that may arise in the treatment process.

• Ensure precise and timely documentation of patient interactions, including clinical notes and progress reports, complying with department guidelines and auditing requirements.

The clientele you will support includes a diverse range of individuals dealing with mental health disorders, trauma, and substance use issues. Qualifications\n

• Minimum of 3 years experience working in an outpatient counseling setting, with a proven track record of effective client management and therapy delivery.

• A Master’s degree in Marriage and Family Therapy, Counseling, Social Work, or Psychology from an accredited institution.

• Current, valid licensure in the state of Oregon as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

• Extensive knowledge of various therapeutic theories, assessment tools, and evidence-based modalities.
\n
• Exceptional verbal and written communication skills, with a focus on clear and effective dialogue with clients and team members.

• Prior experience utilizing telehealth technologies to deliver services effectively in a remote capacity.

• Demonstrated ability to foster collaboration and rapport with interdisciplinary healthcare teams.

Compensation and Benefits\n
