MDS Coordinator

Skilled Nursing Facility - We are seeking an experienced MDS Coordinator to join our skilled nursing facility in Dayton, Ohio. This is a great opportunity for an LPN or RN with strong long-term care experience who understands the MDS process, reimbursement, and regulatory requirements. Responsibilities: Coordinate and complete accurate, timely MDS assessments Manage care planning and interdisciplinary team coordination Monitor documentation for accuracy and regulatory compliance Support reimbursement and quality measure initiatives Participate in audits, surveys, and ongoing quality improvement efforts Collaborate closely with nursing, therapy, social services, and other departments Qualifications: Current Ohio LPN or RN license Previous MDS experience in a skilled nursing or long-term care setting preferred Knowledge of MDS 3.0, RAI guidelines, Medicare/Medicaid, and PDPM PointClickCare experience preferred RAC-CT certification is a plus Strong organizational skills and attention to detail We Offer: Health, dental, and vision insurance Paid time off & Paid Holidays 401(k) Continuing education opportunities Supportive leadership and a collaborative team environment If you are an experienced MDS professional looking for an opportunity to make an impact within a supportive skilled nursing team, we would love to hear from you. #J-18808-Ljbffr Skilled Nursing Facility
