Key Responsibilities\n

• Conduct comprehensive mental health assessments to pinpoint patient needs and recommend appropriate interventions tailored to individuals' circumstances and conditions.

• Utilize standardized assessment tools and engaging clinical interviews to evaluate mental health disorders, substance use issues, and critical social determinants of health.

• Deliver evidence-based therapeutic interventions in various settings, including individual, group, and family therapy, to address a wide array of mental health and behavioral challenges.

• Develop, implement, and regularly update personalized care plans with clear, measurable goals to track client progress and adapt to changing needs.

• Facilitate effective coordination of patient care, ensuring timely referrals to community resources, follow-up appointments, and continuity of care as required.

• Maintain thorough and accurate documentation of all assessments, treatment plans, progress notes, and discharge summaries, ensuring compliance with both regulatory and organizational standards.

Patient Population / Client Focus\n

Your work will primarily involve clients experiencing a range of mental health disorders. These may include anxiety, depression, trauma-related issues, and more. Qualifications\n

• A Master’s Degree in Counseling, Social Work, Marriage & Family Therapy, or Psychology from an accredited educational institution is required.

• Must possess a current Pennsylvania State License as an LCSW, LMFT, or LPC.

• A minimum of three years of recent clinical experience in mental health settings is necessary to ensure a high level of competency in service delivery.
