Key Responsibilities\n

• Conduct thorough assessments to identify the social, emotional, and environmental challenges faced by our clients.

• Develop and implement individualized service plans that outline goals, interventions, and mechanisms for progress tracking.

• Provide counseling to individuals and families experiencing significant life challenges, working to facilitate healing and resilience.

• Collaborate closely with clients to tailor service plans that connect them with essential community resources, ensuring they have the support they need.

• Perform crisis interventions, conducting risk assessments and creating safety plans as necessary to protect our clients’ well-being.

• Engage with various community agencies and organizations to effectively coordinate services, advocate for clients, and address their needs comprehensively.

• Participate in community outreach initiatives to raise awareness about the services offered by Legacy Healing Center.

• Engage in regular case conferences, team meetings, and ongoing staff training to enhance service delivery.

Patient Population / Client Focus\n

You will work primarily with vulnerable populations within a hospice setting, including individuals at the end of life and their families. Your role is vital in addressing emotional distress and providing support during one of the most challenging times in their lives. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n
