Key Responsibilities\n

• Assess and diagnose patients' mental health needs using established diagnostic tools such as the Diagnostic and Statistical Manual of Mental Disorders. Develop individualized treatment plans that respect cultural diversity and specific needs.

• Monitor and evaluate patient progress continually, adapting interventions and strategies to support mental health diagnoses and improve outcomes.

• Prepare and maintain accurate treatment records and documentation using current technology, ensuring compliance with regulatory standards and organizational protocols.

• Collaborate with patients, families, and other clinical team members to formulate comprehensive treatment plans that include achievable goals and desired outcomes.

• Anticipate patient safety concerns and actively participate in a team environment, working collectively towards shared objectives.

• Coordinate referrals and maintain effective communication between patients and various treatment providers, establishing strong professional relationships to enhance patient care.

• Provide education and mentorship to students, interns, residents, and volunteers, fostering a culture of learning and professional development.

Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate program and a valid license as a Licensed Clinical Social Worker (LCSW) from the State Board of Social Work Licensure.
