Summary:

We are seeking a highly skilled Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team at Carease Health - MD. As a hybrid role, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities through a combination of on-site visits and remote work from home. Our dedicated scribe team will provide support for efficient documentation.

Responsibilities:

• Provide preventative care to residents in skilled nursing facilities through on-site visits and remote work from home

• Conduct evaluations, assessments, and treatment plans for patients

• Collaborate with healthcare teams to ensure comprehensive care and support

• Maintain accurate and timely documentation of patient records

• Participate in quality improvement initiatives to enhance patient care and outcomes

• Build strong relationships with facility staff and residents

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Strong problem-solving and critical thinking skills

• Ability to work independently and as part of a team
