Work Environment

Key Responsibilities

In this role, you will:

• Conduct comprehensive evaluations of patients to develop individualized treatment plans that address their unique functional limitations and rehabilitation goals.
• Implement and monitor evidence-based treatment protocols, adapting strategies as necessary based on ongoing assessments of patient progress.
• Maintain thorough documentation of patient evaluations, treatment plans, and progress notes, ensuring compliance with all legal, ethical, and organizational standards.
• Collaborate with fellow healthcare professionals to formulate holistic care strategies, referring patients for additional services as required and recognizing when changes in treatment plans are necessary.
• Engage in proactive discharge planning and coordinate follow-up care to ensure seamless transitions and optimal recovery.
• Adhere to state practice acts and guidelines, and uphold the highest standards of practice and patient safety throughout your care delivery.
• Participate in team meetings and contribute to initiatives aimed at removing barriers to access for patients, as well as improving quality of care across the facility.

Patient Population / Client Focus

Our patients are diverse, ranging from those recovering from orthopedic surgeries to individuals managing chronic pain and functional limitations due to injuries or degenerative conditions. You will have the rewarding opportunity to help patients regain their strength, mobility, and confidence, ultimately facilitating their return to daily activities.

Qualifications

The ideal candidate will possess:

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• State licensure as a Physical Therapist (PT) in New Jersey, as regulated by the Physical Therapy Examining Committee.
• A minimum of two years of outpatient orthopedic experience demonstrating applied clinical skills in treatment modalities.
• Current certification in CPR, with an emphasis on ensuring safety in all patient interactions.

Preferred Experience

While the minimum qualifications include two years of relevant experience, candidates with specialized training or certification in manual therapy or advanced rehabilitation techniques will be viewed favorably. Experience in a fast-paced outpatient setting and familiarity with electronic health record (EHR) systems will greatly enhance your application.
