Key Responsibilities\n

• Conduct individual and group therapy sessions focused on the mental health needs of clients, utilizing treatment modalities appropriate for a community-based setting.

• Provide comprehensive case management services, including psychosocial assessments, development of treatment plans, and coordination of care with other health providers.

• Engage in risk assessment and intervention strategies, actively monitoring and documenting patient status changes while identifying any potential risks to safety.

• Collaborate with a multidisciplinary team, including psychologists, psychiatrists, and administrative staff, to enhance the effectiveness of treatment interventions and address any emerging clinical needs.

• Stay current with all safeguarding policies and procedures; identify any new safeguarding risks and escalate to Clinical Leads promptly for appropriate action.

• Contribute to a culture of continuous improvement by collecting and analyzing client feedback, monitoring treatment outcomes, and making recommendations for program enhancements.

Patient Population / Client Focus\n

Your focus will be on a diverse range of clients including children, adolescents, and adults facing various challenges, such as mental illness and substance use disorders. This role demands a compassionate approach, as you work closely with clients to navigate complex emotional and social issues, including trauma and family dynamics.

Qualifications\n

• Master's Degree in Social Work from an accredited institution.

• Active and valid Licensed Clinical Social Worker (LCSW) certification is required.

• A minimum of two years of professional experience in a behavioral health setting, demonstrating proficiency in individual and group therapy.

• Expertise in conducting comprehensive psychosocial histories, including suicide risk assessments and case management evaluations.
