Work Environment\n

The selected candidate will work primarily in a remote setting aimed at maximizing flexibility while ensuring quality care for clients. While most tasks will be conducted from home, regular communication and collaboration with interdisciplinary teams—including healthcare professionals, caregivers, and administrative staff—will be essential. Candidates should be comfortable utilizing virtual communication platforms and electronic health record (EHR) systems to provide effective care.

Standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend responsibilities to meet the needs of our clients. On-call availability may also be required for urgent situations, demonstrating a commitment to case management and client welfare.

Key Responsibilities\n

• Conduct detailed assessments of clients' psychosocial needs, taking into account emotional, mental, and social factors that impact their overall well-being.

• Develop personalized care plans in collaboration with clients and their families to meet specific goals, preferences, and needs.

• Provide counseling and emotional support, assisting seniors and their families in navigating transitions such as loss, illness, or changes in living situations.

• Advocate for clients, ensuring they have access to essential community resources and services that can aid their social, physical, and emotional health.

• Regularly visit clients to evaluate living conditions, monitor changes in needs, and resolve any safety or care issues promptly.

• Collaborate with interdisciplinary teams to enhance client care and ensure a holistic approach to service delivery.

• Educate clients and families on community resources, support groups, and programs that promote healthy social engagement.

• Maintain accurate and comprehensive documentation of client interactions, assessments, and care plan progress in compliance with professional standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
