CASE MANAGER (LMSW)

Responsibilities JOIN OUR TEAM! Spring Mountain Treatment Center is currently seeking a dynamic professional to join our team as a PRN Licensed Case Manager. The Case Manager provides quality case management and psychotherapeutic services to Spring Mountain’s populations. They serve as a member of a multi-disciplinary team supporting the hospital’s treatment program and philosophy and assure the delivery of quality treatment to patients and their family members. The Case Manager provides social histories, discharge planning, case management and psycho-educational/process groups to age specific populations. JOB DUTIES/RESPONSIBILITIES: Able to relate and communicate positively, effectively and professionally with others; work calmly and respond courteously under pressure. Attends educational sessions to continually update knowledge base related to behavioral health and social work issues. Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members. Coordinates referrals for services and utilizes interpersonal skills to maximize excellent customer service, safe patient care and professionalism. Works with Discharge Planner to develop, implement and document a safe discharge plan. Provides group and family process-oriented therapies as well as conducts educational and other groups for patients and family members using various professional treatment modalities. Performs other duties as assigned. ABOUT SPRING MOUNTAIN TREATMENT CENTER: Spring Mountain Behavioral Services has provided mental health services to the Las Vegas community since 2001, treating children, teens, adults and senior adults with behavioral health or addiction issues. Our primary location, Spring Mountain Treatment Center, focuses on the stabilization of acute symptoms for patients age 5 to 65, along with select outpatient care programs for teens. Spring Mountain Treatment Center is a 110-bed mental health facility that provides acute crisis stabilization services for adults, teens, and children in a secure inpatient setting.
