Fairview is seeking an Internal Medicine Physician to join our primary care team at M Health Fairview Clinic Midway . This is an excellent opportunity to join a thriving practice with multiple on-site specialties and a long-tenured, highly experienced support staff.

Practice Details:

• 12 provider primary care practice with more than 7 specialists onsite.
\n
• Patient encounters average 18-22 patients per day.
\n
• 34 patient contact hours per week.
\n
• Call Frequency: 1:18 low intensity - phone consultation only with first tier nurse triage.
\n
• As part of Fairview s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
\n
• Openings: 0. 8 to 1.0 FTE (Full Time) - Flexible schedule.
\n
• Fully integrated Electronic Medical Record (EPIC).
\n

Qualifications:

\n
• Graduate of an accredited ACGME/AOA Internal Medicine residency training program.
\n
• Board Certified/Board Prepared with the American Board of Medical Specialties or American Osteopathic Association.
\n
• Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.
\n
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
\n
• Meet threshold criteria/qualifications for Credentialing and Privileges.
\n

$305,787 - $339,764 Annual *based on full time (1.0 FTE)
