Key Responsibilities\n

• Engage with clients to identify their personal goals and preferences through one-on-one sessions prior to drafting personalized rehabilitation plans.
\n
• Develop and implement comprehensive rehabilitation plans that consider clients' education levels, aptitudes, physical abilities, and future aspirations, adapting as necessary throughout the course of treatment.
\n
• Collaborate closely with a dynamic team of physicians, occupational therapists, psychologists, and other mental health professionals to design and execute tailored rehabilitation programs that meet each client's unique needs.
\n
• Maintain detailed case files and documentation, including personal information, eligibility, insurance details, the services provided, and ongoing narrative accounts of client progress.
\n
• Monitor and assess clients' progress towards their rehabilitation goals, regularly reviewing and adjusting plans as needed to ensure optimal outcomes.
\n
• Stay in constant contact with clients as they participate in job training, occupational placement programs, and ensure timely interventions for any challenges or issues that arise.
\n
• Engage with clients’ families and support networks to evaluate how best to meet the clients' needs effectively and collaboratively.
\n
• Conduct thorough needs analyses and create timely documentation for all assessments and recommendations.
\n
• Offer referrals and resource recommendations that align with the clients' comprehensive care plans.

Patient Population / Client Focus\n

In this role, you will primarily work with clients who are navigating mental health challenges, including mood disorders and other psychological conditions. You'll engage in meaningful therapeutic relationships, encouraging progress and independence through innovative rehabilitation strategies tailored to individual client circumstances.

Qualifications\n

• A Master’s Degree in a relevant discipline such as counseling, social work, psychology, or marriage and family therapy is required.

• Individuals must hold a valid Licensed Professional Counselor (LPC) license.
\n
• Experience working with clients facing mental health disorders in either residential or outpatient settings is essential.

• A collaborative attitude and a passion for fostering positive change in clients’ lives are crucial.
\n
• Robust clinical skills in motivational interviewing, cognitive-behavioral therapy, dialectical behavioral therapy, and effective documentation practices are highly regarded.
