Apex Physical Therapy is seeking a dedicated and compassionate Full-Time Physical Therapist (PT) to join our home care team in Hampton, Virginia. Work Environment

This full-time role is primarily onsite, as you will be visiting patients in their homes to provide therapeutic services. You will work closely with an interdisciplinary team of healthcare providers, collaborating to ensure each patient receives the best possible care in a comfortable and supportive setting. Your weekly schedule will consist of 40 hours, with some variable weekends, providing flexibility while ensuring comprehensive service delivery.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients to determine functional limitations and establish detailed treatment plans tailored to their unique needs.

• Provide expert consultations to healthcare colleagues regarding patients' continuity of care, ensuring seamless transitions and adherence to the treatment protocols.

• Foster respectful and effective interactions with patients of varying ages and backgrounds, embodying our high customer service standards in every engagement.

• Document and implement treatment plans accurately, performing regular evaluations and adjustments based on patient progress and feedback.

• Maintain compliance with all laws, regulations, and professional standards in documentation, treatment methodologies, and discharge planning.

• Communicate effectively within the treatment team and with referral sources, ensuring all parties are informed of the patient's status and any necessary changes to their care plan.

• Engage in community outreach and education initiatives, participating in continuous quality improvement (CQI) efforts and helping to market the services of Apex Physical Therapy.

• Attend staff meetings and professional development activities, remaining abreast of the latest best practices in physical therapy and treatment modalities.

Patient Population / Client Focus

As a Physical Therapist at Apex, you will work with a diverse clientele, including elderly patients recovering from surgery, individuals with chronic conditions, and those recuperating from injuries. Your role is pivotal in helping these individuals regain independence and improve their quality of life through tailored rehabilitation strategies and ongoing support.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.

• A valid State License in Physical Therapy is required to practice, ensuring adherence to local regulations and standards.

• Basic Life Support (BLS) certification is mandatory, reflecting our commitment to patient safety.

• Strong written and verbal communication skills are essential for effective patient interactions and collaboration with team members.

• Proficiency in computer skills, particularly word processing and medical documentation software, is necessary for maintaining up-to-date patient records.

Preferred Experience

• Prior experience in home health care or a related field is highly desirable, as it equips candidates with the necessary skills to address the unique challenges faced by patients in their home environments.

Compensation and Benefits

Apex Physical Therapy offers a competitive salary range of USD 100,000 – 120,000 annually, complemented by a comprehensive benefits package designed to support your well-being.
