Work Environment

This position is fully onsite, requiring the Physical Therapist to travel to various patient homes within the Hampton area. You will work primarily Monday to Friday, with occasional weekend shifts on a rotating basis to ensure comprehensive patient care. Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical capabilities and limitations, integrating findings to develop effective and individualized treatment plans.
• Implement treatment strategies according to physician directives, ensuring that interventions are tailored to meet patient-specific goals, functional needs, and preferences.
• Maintain accurate and timely documentation of patient care activities, including evaluations, progress notes, treatment protocols, and discharge summaries, using common clinical terminology.
• Monitor patient progress and therapeutic efficacy by regularly assessing treatment outcomes and making necessary adjustments to intervention approaches.
• Provide mentorship and supervision to Physical Therapist Assistants (PTAs) and support staff, ensuring an effective team dynamic that enhances patient care delivery.
• Collaborate closely with an interdisciplinary team of healthcare professionals—including physicians, occupational therapists, speech-language pathologists, and nursing staff—to coordinate patient care and share insights on treatment plans.
• Participate in ongoing professional development and training initiatives, contributing to the enhancement of therapy practices and patient care methodologies.

Patient Population / Client Focus

You will primarily work with patients recovering from surgery, managing chronic conditions, or rehabilitating from injuries. The patient demographic spans various age groups, requiring you to adapt your therapeutic techniques to cater to both young and older patients, focusing on their unique physical, emotional, and social needs.

Qualifications

• Must hold a Bachelor of Science degree in Physical Therapy from an accredited university.
• Active State licensure as a Physical Therapist in Virginia is required.
• Strong skills in both verbal and written communication to facilitate effective patient and team interactions.
• Proficiency with electronic health records and basic computer skills are essential for maintaining documentation and accessing patient information.

Preferred Experience

While a minimum of two years of relevant experience in a therapeutic setting is preferred, candidates with a strong passion for patient care and a commitment to ongoing education will be considered. Candidates with specialized training in geriatric or neurological rehabilitation may receive priority.

Compensation and Benefits
