Community Liaison

Radiance Home Health is seeking a driven and relationship focused Community Liaison to join our growing team.

This role is responsible for building strong relationships with hospitals, case managers, and community partners to generate patient referrals and support agency growth.

Responsibilities:

• Develop and maintain relationships with hospitals, rehab centers, and referral sources
• Actively generate and track patient referrals
• Conduct in-person visits to healthcare facilities
• Educate referral sources on Radiance services
• Follow up consistently with new and existing contacts
• Collaborate with internal clinical and intake teams

Qualifications:

• 2+ years experience in healthcare marketing, liaison, or home health
• Strong communication and relationship-building skills
• Self-motivated and goal-oriented
• Ability to work independently and manage schedule
• Knowledge of home health services (preferred)

Compensation:

• Paid Per Qualified Lead Plus Hourly
• Monthly Referral performance bonuses
