Work Environment

This full-time position is based onsite, directly in the homes of patients throughout the Orlando area, ensuring that we meet their rehabilitation needs in the comfort of their own environment. Key Responsibilities

• Conduct thorough evaluations of patients, taking detailed histories and performing standardized assessments to determine their physical therapy needs and develop customized treatment plans.
• Communicate evaluation findings to patients and their families clearly, ensuring they understand the treatment plan and engage actively in the goal-setting process.
• Implement and modify treatment protocols based on patient evaluations and progress, demonstrating proficiency in various therapeutic techniques tailored to individual needs.
• Collaborate with medical professionals and other healthcare providers to ensure comprehensive patient care, recognizing when modifications to the care plan are needed and documenting all assessments and interventions accurately.
• Execute appropriate discharge planning and continuity of care strategies, ensuring that all documentation meets regulatory compliance and reflects the patient's treatment journey.
• Manage time effectively to satisfy productivity expectations, maintain an accurate schedule, and contribute positively to team marketing initiatives within the community.
• Contribute to departmental goals, engage in quality improvement activities, and comply with all governmental and accreditation standards, enhancing the overall delivery of care.

Patient Population / Client Focus

You will work with a diverse and varied patient population, including individuals recovering from surgery, those with chronic conditions, and elderly patients needing general rehabilitation services. Qualifications

• A Bachelor's Degree in Physical Therapy or a related field is required.
• A valid physical therapy license in the state of Florida.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Strong interpersonal skills and a commitment to delivering exceptional patient care.

Preferred Experience

• At least 2 years of clinical experience in a physical therapy setting, with a preference for experience in home health care.
• Prior exposure to creating and adapting patient treatment plans to address individual needs effectively.

Compensation and Benefits
