Work Environment

This position requires an onsite presence, mainly focusing on home care for patients. Key Responsibilities

• Conduct comprehensive patient evaluations by reviewing medical history, performing physical assessments, and determining individualized treatment plans that align with best practices.

• Implement evidence-based treatment protocols that reflect the patient's current health status, ensuring modifications as required based on progress and changing conditions.

• Collaborate effectively with physicians and other healthcare providers to discuss evaluation results, treatment decisions, and necessary adjustments to patient care plans.

• Maintain accurate and timely documentation of patient evaluations, treatment logs, and progress reports in compliance with healthcare regulations, ensuring a clear narrative of the patient's journey through therapy.

• Engage in ongoing communication with patients and their families, patiently explaining therapeutic interventions and setting realistic recovery goals.

• Participate in departmental meetings and training sessions to stay abreast of clinical best practices and regulatory updates, enhancing personal and team development.

• Manage discharge planning and follow-up care protocols to promote continuity of care and ensure successful rehabilitation of patients.

Patient Population / Client Focus

In this role, you will primarily serve orthopedic patients in a home care environment. This includes individuals recovering from surgeries, managing chronic pain, or dealing with injuries that impact their mobility and quality of life. Your expertise will enable patients to regain strength and functionality, guiding them through personalized therapeutic strategies that enhance their rehabilitation process in a familiar, comfortable setting.

Qualifications

• A Bachelor's degree in Physical Therapy or an equivalent degree as recognized by the State of New Jersey is required.

• A valid state license to practice as a Physical Therapist is essential.

• Current American Heart Association's BLS for Healthcare Providers certification is mandatory.

• Strong interpersonal skills and proven ability to communicate effectively with diverse populations.

Preferred Experience

While a minimum of 2 years of experience as a Physical Therapist in an orthopedic setting is required, candidates with additional certifications in specialized therapeutic techniques or experience in home care will find this role particularly fitting. Compensation and Benefits
