Key Responsibilities

• Conduct comprehensive evaluations upon patient intake, which include gathering pertinent medical history, performing detailed assessments, and developing tailored treatment recommendations.
• Clearly communicate evaluation findings and treatment strategies to patients and their families, ensuring their active participation in the goal-setting process.
• Demonstrate proficiency in implementing individualized treatment protocols based on therapist evaluations, evolving healthcare trends, and evidence-based practices.
• Collaborate with a multidisciplinary team of healthcare professionals, ensuring comprehensive care and adjacently addressing patient needs.
• Monitor and document changes in patients' conditions that may necessitate adjustments to care plans or additional medical consultations. Maintain meticulous records to track patient progress.
• Oversee discharge planning and provide follow-up care to ensure continuity and quality of treatment as patients transition out of therapy.
• Meet productivity expectations while managing time effectively, attending meetings punctually, and participating in community events aimed at promoting physical health.
• Contribute to departmental goals by actively engaging in quality improvement initiatives and compliance with healthcare regulations.
• Engage in professional development activities, including continuing education and serving as a clinical instructor for physical therapy students.

Qualifications

Compensation and Benefits
