Home Health Occupational Therapist

About the Role Help patients regain independence and return to the daily activities that bring meaning to their lives. As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes, using creativity, clinical expertise, and problem-solving to help them rebuild the skills needed for everyday living. From dressing and bathing to cooking, managing routines, and engaging in meaningful activities, you'll guide patients as they rediscover their ability to live safely and independently. This role is ideal for a licensed Occupational Therapist who thrives in a flexible, patient-centered environment and enjoys tailoring care to each individual's unique goals and home setting. You'll assess functional abilities, design personalized treatment plans, and develop adaptive strategies and solutions that support independence and safety. Working closely with patients, families, and an interdisciplinary care team, you'll provide education, hands‑on training, and encouragement as patients progress toward greater confidence and independence in their daily lives. Location Office Location: Jackson Main Service Area: Jackson, MI and surrounding This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes Hours Monday to Friday, 8 a.m. to 5 p.m. Weekend On‑Call Rotation Required Qualifications Graduate of an accredited occupational therapy program (ACOTE accredited) Current state license as an Occupational Therapist Minimum 1 year of experience as a licensed Occupational Therapist Reliable transportation, valid driver’s license, and valid automobile insurance coverage Willingness to travel daily for home visits within service area Preferred Qualifications Home health experience Experience with adaptive equipment and home modifications Knowledge of Medicare home health documentation Essential Skills and Competencies Strong clinical assessment and treatment planning abilities Knowledge of medical terminology and home health regulations Excellent problem‑solving and creative thinking skills Strong verbal and written communication abilities Ability to work independently with minimal supervision Organizational skills and attention to detail Comfort working in varied home environments Interpersonal skills for professional interaction with patients, families, and teams Pay Range $84,000—$94,000 USD How We Care for You Minimum of 3 Weeks Paid Time Off (PTO) Medical, Dental, and Vision Insurance HSA and FSA options including Dependent Care Company paid Short Term Disability Company paid Life Insurance 401(k) with Employer Match Mileage Reimbursement Company Vehicle Program for field roles Pet Insurance ID and Fraud Protection Background Screening Employment is contingent upon the successful completion of a background check.
