Key Responsibilities\n

• Conduct thorough clinical assessments for patients entering hospice care, ensuring adherence to best practices and ethical guidelines.

• Develop individualized treatment plans that respect patients' end-of-life preferences while addressing their emotional and psychological needs.

• Utilize evidence-based therapeutic interventions, such as cognitive behavioral therapy and supportive counseling, to assist patients and their families.

• Maintain accurate and up-to-date clinical documentation and patient records in compliance with HIPAA regulations and organizational policies.

• Collaborate with a multidisciplinary team, including nurses, physicians, and chaplains, to ensure comprehensive and cohesive care delivery.

Qualifications\n
