Work Environment

As a Physical Therapist in our home care program, you will engage directly with patients in their homes, providing personalized therapy in a familiar and comfortable environment. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, assessing functional abilities and limitations to develop individualized treatment plans.
• Implement and modify therapeutic exercises, manual therapy techniques, and modalities to enhance recovery and improve patients' functional performance.
• Monitor, document, and analyze patient progress, ensuring adjustments to treatment plans based on clinical outcomes and patient feedback.
• Educate patients and their families about their conditions, treatment options, and home exercise programs designed to reinforce therapy between clinical visits.
• Stay abreast of advances in orthopedic therapy, incorporating the latest evidence-based practices into your patient care strategies.
• Collaborate with other healthcare professionals to ensure comprehensive care, facilitating communication with physicians, nurses, and occupational therapists regarding patient needs.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population that includes individuals recovering from orthopedic surgeries, sports injuries, and chronic musculoskeletal conditions. Our focus is to provide holistic care tailored to each patient's unique situation, helping them navigate their recovery journey with empathy and expertise.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited program.
• A current and valid Physical Therapist license in the state of New Jersey.
• A demonstrated commitment to high-quality, patient-centered care in previous roles.
• Strong interpersonal skills to effectively communicate with patients and their families, along with a capacity for working collaboratively within a multidisciplinary team.
• An ability to thrive in a home care environment, managing your schedule and patient interactions autonomously.

Preferred Experience

• Minimum of three years of experience as a Physical Therapist, specifically in orthopedic rehabilitation.
• Experience with home care services and a thorough understanding of home safety assessments and modifications.
• Familiarity with electronic health record (EHR) systems for documentation and patient management.

Compensation and Benefits

We offer a competitive salary ranging from USD 110,000 to 130,000 annually, commensurate with your experience and expertise.
