Join Pulse Point Internal Medicine as a dedicated Nurse Practitioner, where you will play a crucial role in providing outstanding primary care to Medicare-eligible adults. This full-time position offers a competitive salary range of USD 180,000 – 200,000 annually, depending on experience, and focuses on delivering personalized, value-based care directly within the community.

Work Environment

This position is based onsite, working collaboratively within a supportive outpatient practice team that includes physicians, fellow nurse practitioners, physician assistants, medical assistants, registered nurses, and care managers. You will have a dedicated medical scribe to assist with documentation, allowing you to focus on patient care, and will collaborate with a network of elite specialists and hospitals for coordinated acute and specialty care.

Key Responsibilities

• Deliver exceptional primary care by conducting comprehensive evaluations and follow-up visits for both routine and acute medical issues.
• Lead annual wellness visits and health risk assessments, integrating a holistic perspective of patient health while ensuring precise documentation.
• Engage in care coordination across different providers, specialists, and agencies to enhance patient care pathways.
• Play a pivotal role in population health initiatives, ensuring that all eligible patients receive necessary screenings, such as biannual breast cancer assessments.
• Support the care team through phone triage, outreach, and patient education on health conditions, treatment options, and care plans.
• Partake in marketing and promotional activities to strengthen community awareness of our innovative care model.
• Conduct home visits as required to provide personalized care in the patient's environment.
• Report directly to the Medical Director and collaborate closely with operational leadership to enhance clinical practices.

Patient Population / Client Focus

Your primary focus will be on older adults who are enrolled in Medicare, ensuring that their unique healthcare needs are met through proactive, compassionate care. Your practice will emphasize preventive measures and long-term health management, allowing patients to live vibrant and fulfilling lives within their communities.

Qualifications

• A Master of Science in Nursing, a Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Prior experience in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Comprehensive understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A strong team-oriented mindset and a commitment to collaborative cross-functional work.

Preferred Experience

• Bilingual proficiency is preferred, enhancing communication with a diverse patient population.
