Key Responsibilities

• Conduct thorough evaluations of patients presenting with various cardiovascular conditions, including hypertension, heart failure, and arrhythmias.
• Develop, implement, and modify patient-specific treatment plans in collaboration with cardiologists, nurses, and other healthcare professionals.
• Educate patients and their families on treatment plans, medication management, and lifestyle changes necessary for improving heart health.
• Utilize advanced diagnostics, including echocardiograms and stress tests, to assist in forming a comprehensive understanding of each patient’s condition.
• Regularly monitor patient progress, adjusting treatment regimens as required, and document findings accurately in electronic health records.
• Participate actively in multidisciplinary team meetings to ensure integrated care approaches and address all aspects of patient health.
• Stay abreast of the latest research and advancements in cardiology to provide evidence-based care for our patients.

Patient Population / Client Focus

Our patient population mainly comprises residents of the assisted living facility, many of whom are older adults with chronic cardiovascular conditions. As a Cardiology Nurse Practitioner, you will engage with these individuals, ensuring not only their medical needs are met but also promoting their autonomy and emotional health through education and support.

Qualifications

• Master of Science in Nursing (MSN) with a concentration in cardiology or completion of a Family Nurse Practitioner program that includes significant cardiology coursework.
• Current certification as a Cardiology Certified Nurse Practitioner (CCNP) is highly desirable.
• A minimum of two years experience as a Nurse Practitioner, specifically in cardiology, primary care, or related fields.
• Strong interpersonal skills with the ability to build rapport with patients and collaborate effectively with a diverse healthcare team.
• Proficiency in utilizing electronic health record systems and other healthcare technologies.

Preferred Experience

While two years of cardiology-related experience is required, candidates with additional experience in geriatrics or chronic disease management will be given preference, as these skills are crucial in addressing the unique needs of our patient demographic.

Compensation and Benefits
