Expert Cares Healthcare Services, LLC is seeking a dedicated and compassionate Licensed Nurse Practitioner (NP) to join our collaborative team in Buffalo, New York. Key Responsibilities

• Provide comprehensive health education to patients and families, ensuring they understand their care plans and treatment options.
• Create and articulate personalized treatment plans, clearly communicating these to patients and relevant caregivers.
• Order and interpret lab and diagnostic tests, adjusting treatment strategies based on findings to ensure optimal patient outcomes.
• Maintain thorough and compliant documentation within our EMR system to support ongoing patient care and legal standards.
• Collaborate continuously with local and regional operational and clinical leadership to meet corporate goals and improve service delivery.
• Support strategic initiatives surrounding value-based care, aiming to enhance patient satisfaction and health outcomes.
• Ensure delivery of high-quality customer service, fostering lasting relationships with patients and their families.
• Practice in accordance with ethical standards and the Scope and Standards of Practice of the Nurse Practitioner profession.
• Adhere to all relevant state and federal healthcare regulations and guidelines.
• Engage with primary care providers and interdisciplinary care teams to create cohesive care plans for residents.
• Coordinate care effectively with specialists, including utilizing telemedicine services when necessary.
• Complete regulatory-required visits on a timely basis, providing both routine and acute care as needed.
• Participate in family conferences and advanced care planning discussions to ensure comprehensive support.
• Address any pharmacist recommendations to optimize medication management for residents.
• Be present in assigned facilities during work hours to guarantee adequate on-site availability for patient care needs.
• Assess, diagnose, and manage a variety of chronic and acute health conditions, tailored to the senior population.

Patient Population / Client Focus

This role primarily focuses on seniors and other underserved populations, including individuals residing in assisted and independent living communities, as well as those in skilled nursing facilities. Understanding the unique healthcare challenges faced by these demographics is essential for providing individualized, compassionate care.

Qualifications

• Must possess national certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Hold an active, unrestricted Nurse Practitioner state license.
• Have a valid and active DEA license.
• Must be authorized to work in the United States.
• Exhibiting flexibility in work environment and scheduling is essential to respond to the dynamic needs of the patient population.
• Demonstrated passion for delivering healthcare services to seniors and low-income individuals.

Preferred Experience

• A graduate degree from an accredited Master of Science in Nursing (MSN) program, Doctor of Nursing Practice (DNP), or Nurse Practitioner certificate program is required.
• Experience in geriatric care or outpatient settings is a significant advantage.

Compensation and Benefits
