Work Environment

This role will be performed onsite at a local school, allowing you to interact intimately with students and staff while cultivating a sense of community. Key Responsibilities

• Conduct thorough assessments of students’ health needs, including physical exams, patient histories, and diagnostic testing.
• Develop and implement personalized care plans in collaboration with patients, families, and interdisciplinary teams.
• Provide education on health promotion, wellness, and disease prevention to students and their families.
• Diagnose and treat acute and chronic illnesses, ensuring that follow-up care is effectively communicated and executed.
• Maintain up-to-date patient records in accordance with established guidelines and regulatory requirements, utilizing electronic health records systems efficiently.
• Engage in community outreach initiatives to promote health education and resources available to families.

Patient Population / Client Focus

This role will primarily involve working with school-aged children, including those from diverse backgrounds and varying health statuses. You will encounter a range of cases from routine check-ups and immunizations to addressing chronic health conditions and mental health concerns. Your ability to engage with children, parents, and healthcare teams will foster a supportive atmosphere conducive to effective healthcare delivery.

Qualifications

• A Master's degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner, with clinical competencies in pediatric and adolescent care preferred.
• An active nursing license in the state of New York.
• A minimum of three years of clinical experience, ideally within community health or school health settings.

Preferred Experience

• Experience in a school-based health program or community outreach role.

Compensation and Benefits

The position offers a competitive salary range of $150,000 to $160,000 annually, commensurate with experience and qualifications.
