Work Environment

Located in Buffalo, NY, our facility is a modern hospital that provides a collaborative and supportive environment for healthcare professionals. Nurse Practitioners will work onsite, engaging directly with patients and interdisciplinary team members in a dynamic setting designed for optimal patient care. Our schedule runs from Monday to Friday, 9 AM to 5 PM, promoting a work-life balance that allows for both dedication to patients and personal time.

Key Responsibilities

• Perform comprehensive assessments of patients, including medical history reviews and physical examinations to determine their health status.
• Manage patients through acute medical situations or exacerbations of chronic conditions by ordering diagnostic tests, interpreting results, and providing appropriate interventions.
• Guide patients through short-term sub-acute rehabilitation episodes, utilizing best practices to optimize recovery.
• Ensure meticulous documentation of patient encounters, including coding with ICD-10 to maintain accuracy in medical records.
• Collaborate effectively with the interdisciplinary care team, sharing insights and updates on patient conditions to provide cohesive care.
• Engage in advanced illness conversations, offering patients and caregivers essential counseling and education about care options and end-of-life discussions.
• Utilize electronic medical records systems to document and track patient information consistently and effectively.

Patient Population / Client Focus

Qualifications

• Current Nurse Practitioner certification.
• Active state nursing license in New York.
