Work Environment

This position is based onsite at our Assisted Living Facility in Buffalo, NY. You will work closely with a multi-disciplinary team, including nurses, physicians, social workers, and therapists, to deliver integrated care. The facility is designed to provide a comfortable living environment, ensuring that our residents receive the support and resources they need. Your typical working hours will be from 8:00 a.m. to 5:00 p.m., amounting to a full 40-hour work week.

Key Responsibilities

• Conduct comprehensive assessments of patients and formulate diagnoses to understand both acute and chronic medical conditions.
• Create individualized treatment plans that are tailored to the specific needs and preferences of each patient.
• Prescribe medications, therapies, and necessary diagnostic tests as part of the treatment process.
• Educate residents on disease prevention, healthier living choices, and disease management strategies.
• Collaborate with the interdisciplinary team to ensure seamless coordination of care and appropriate referrals when needed.
• Maintain precise electronic health records (EHR) to support clinical workflows and ensure compliance with regulations.
• Engage in quality improvement initiatives to enhance service delivery and patient outcomes.
• Stay updated on current healthcare regulations, practices, and treatment methodologies to ensure the best patient care.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve residents of our Assisted Living Facility, a demographic often requiring both acute care and ongoing management of chronic ailments. You will engage with a diverse group of patients, including elderly individuals, each with their own unique health histories and needs. Your role will significantly enhance residents' overall well-being through consistent and thorough care.

Qualifications

• Must hold a current certification as a Nurse Practitioner.
• An active New York state nursing license is required.
• Bachelor's degree in nursing is essential; a Master's degree is preferred.

Compensation and Benefits
