Work Environment

This position is based at our onsite facility located within a school setting in Buffalo, NY. Our clinic operates in a collaborative workspace specifically designed for outpatient services, providing a supportive atmosphere where you can thrive. As part of our team, you will work closely with interdisciplinary professionals, utilizing shared resources to ensure comprehensive patient care is delivered efficiently. Your work schedule will be Monday through Friday, from 9 AM to 5 PM, allowing for a balanced work-life integration.

Key Responsibilities

As a Nurse Practitioner at ProMedication Counseling Center, your day-to-day responsibilities will encompass a variety of critical tasks to support patient care:

• Conduct comprehensive health assessments that consider the physical, emotional, and social aspects of patient wellness.
• Manage acute medical situations and exacerbations of chronic conditions by implementing appropriate diagnostic tests and interventions.
• Oversee members during short-term sub-acute rehabilitation episodes, ensuring a smooth transition through care pathways.
• Maintain precise and thorough documentation of patient conditions, utilizing ICD-10 coding for accurate record-keeping.
• Collaborate and communicate with the interdisciplinary care team to streamline patient care processes and enhance outcomes.
• Engage in advanced illness discussions and assist in care planning to support patients and their families during critical moments.
• Provide educational counseling to empower patients and caregivers, highlighting self-management and healthy lifestyle practices.

This role primarily focuses on serving diverse patients within the Buffalo, NY area. You'll work with individuals experiencing a range of health issues, including chronic illnesses and complex behavioral needs. Qualifications

The successful candidate must meet the following requirements:

• Current Nurse Practitioner certification.
• An active nursing license specific to the state of New York.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $130,000 annually, complemented by an extensive benefits package.
