Work Environment

This full-time position is based in Buffalo, New York, within a school-based health center setting. Our modern facility employs a collaborative approach, with physicians, nursing staff, and allied health professionals working together to provide seamless care to our patient population. The typical schedule is Monday through Friday, 9 AM to 5 PM, with occasional rotational weekend and evening responsibilities.

Key Responsibilities

As a Nurse Practitioner at Vital Her, your day-to-day responsibilities will include:

• Conducting thorough initial patient histories and performing comprehensive physical examinations.
• Formulating, documenting, and communicating individualized plans of care while ensuring follow-up to guarantee complete screening and patient preparedness.
• Performing daily patient rounds, offering assessments and ongoing management tailored to each woman’s health needs.
• Issuing orders for medications, laboratory work, and diagnostic testing, along with interpreting results to guide treatment options.
• Collaborating with attending physicians and other healthcare providers to ensure optimal patient care and outcomes.
• Documenting discharge summaries and coordinating referrals while providing patients with clear post-discharge care instructions.
• Adhering to all relevant procedures and compliance guidelines, recognizing that responsibilities are essential functions under the Americans with Disabilities Act.

Patient Population / Client Focus

Your focus will be primarily on women ranging from adolescence to maturity, addressing a spectrum of health challenges that include reproductive health, chronic conditions, mental health concerns, and regular wellness checks. You will leverage a compassionate approach, fostering a safe environment where patients feel comfortable discussing their health issues openly.

Qualifications

To be successful in this role, candidates are required to have:

• A Master’s Degree in a Nurse Practitioner Program.
• National Board Certification in your specialty area.
• A current New York State Nurse Practitioner license and a Registered Professional Nurse license.
• A collaborative practice agreement with designated protocols filed with the NYS Department of Education or a collaborative relationship attestation compliant with the Nurse Practitioner Modernization Act of 2015.

Preferred Experience

Ideal candidates will possess at least one year of experience in a clinical setting, preferably in women's health or a related specialty. Familiarity with electronic health records (EHR) systems and the ability to utilize telehealth technology can also be advantageous.
