Brightwood Orthopedic and Family Medicine Care is seeking a dedicated and passionate Family Medicine Nurse Practitioner to join our team in Buffalo, New York. This full-time position will require the selected candidate to provide exceptional primary care to a diverse adult patient population, primarily those on Medicare, ensuring they receive the highest quality healthcare tailored to their individual needs. Work Environment

The position is based onsite at our hospital facility in Buffalo, NY. You will be part of a collaborative healthcare team that includes physicians, nurse practitioners, physician assistants, registered nurses, care managers, dedicated medical scribes, and supporting administrative staff. This multi-disciplinary team works closely together to ensure seamless care coordination and improved patient experiences. Your role will focus primarily on outpatient practices, allowing you to engage with patients in a comfortable and accessible setting.

Key Responsibilities

As a Family Medicine Nurse Practitioner, you will take on a variety of responsibilities that are crucial to patient care and organizational success:

• Provision of outstanding primary healthcare services to adult patients.
• Conducting office visits for both routine care and urgent health issues.
• Administering annual wellness visits and comprehensive health risk assessments while ensuring precise documentation.
• Coordinating patient care effectively with other healthcare providers, specialists, and relevant agencies.
• Leading population health initiatives, including ensuring compliance with preventive screenings and other evidence-based care measures.
• Assisting the care team with phone triage, patient outreach, and follow-up communications.
• Educating patients on their health conditions, care plans, and treatment options.
• Participating in promotional activities to raise awareness about health and wellness.
• Conducting home visits as warranted by patient needs.
• Reporting to the Medical Director and collaborating closely with operational leadership to optimize care delivery.

Patient Population / Client Focus

Our primary focus is on providing personalized care to older adults covered by Medicare. Qualifications

To be considered for this position, applicants must meet the following qualifications:

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through reputable boards such as AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Active DEA license for prescribing medications.
• Previous experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.

Preferred Experience

The ideal candidate will also demonstrate:

• An understanding of Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and health risk assessments (HRAs).
• A strong sense of teamwork and genuine enthusiasm for collaboration across disciplines.

• Bilingual proficiency in relevant languages is a plus.

Compensation and Benefits

We offer a competitive salary range of USD 115,000 to 150,000 annually, commensurate with experience and expertise. Benefits associated with this role include:
