Key Responsibilities

• Conduct thorough physical examinations and health assessments for patients, ensuring a holistic understanding of their health needs.
• Diagnose and manage acute and chronic illnesses across the lifespan, applying evidence-based practices to inform your clinical decisions.
• Prescribe medications when appropriate and develop personalized treatment plans to meet individual patient requirements.
• Administer immunizations and vaccinations, educating patients about their benefits and any potential side effects.
• Engage in patient counseling, targeting health promotion and disease prevention strategies, as well as providing guidance on lifestyle modifications.
• Order, perform, and interpret necessary diagnostic tests, integrating findings into your treatment plans.
• Collaborate with a multidisciplinary team, including physicians, nurses, and specialists, to coordinate comprehensive patient care.
• Maintain meticulous electronic medical records, ensuring data integrity and accessibility for continuous care.

Patient Population / Client Focus

As a Family Nurse Practitioner with Astoria Health, you will work with patients of all ages—from infants to the elderly—ensuring a family-oriented approach to care. Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Current and active Nurse Practitioner license in the state of New York.
• Certification as a Family Nurse Practitioner.

Compensation and Benefits
