Work Environment

This Nurse Practitioner position is based onsite in our outpatient office located in East Amherst, NY. You will be part of a collaborative care team that includes physicians, physician assistants, medical assistants, and care managers. Key Responsibilities

• Provide exceptional primary care to patients with a focus on preventive health and chronic disease management.
• Conduct thorough office visits that address both routine and acute medical issues, ensuring comprehensive patient assessments.
• Administer annual wellness visits and health risk assessments that require detailed documentation and a holistic approach to health.
• Coordinate care with specialists, testing facilities, and external agencies for seamless patient experiences and better health outcomes.
• Lead population health initiatives, such as ensuring eligible patients receive timely breast cancer screenings and other vital preventive health services.
• Assist the care team in phone triage and outreach efforts, providing guidance and support to patients as needed.
• Educate patients about their health conditions and treatments, helping individuals navigate their care plans effectively.
• Participate in promotional activities as required to enhance the practice's outreach.
• Conduct home visits when necessary to monitor patient health in their own environments.
• Perform other duties as assigned while reporting directly to the Medical Director and working closely with operational leadership.

Patient Population / Client Focus

The primary focus of this role is on adults, particularly older individuals enrolled in Medicare. Our approach emphasizes personalized, value-based primary care aimed at maintaining health and well-being, thereby preventing hospitalizations. You will support a diverse patient population, fostering relationships that prioritize engaging with their medical needs through innovative healthcare solutions.

Qualifications

• A Master of Science in Nursing or Doctor of Nursing Practice, with a specialization in Family, Adult, or Adult-Gerontology Primary Care.
• Active certification from a recognized body (AANP or ANCC) as a Family or Adult Nurse Practitioner.
• A valid, non-probationary state Nurse Practitioner license.
• A current DEA license to prescribe medications.
• Comprehensive understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.

Preferred Experience

• Experience in primary care settings such as internal medicine, family practice, or geriatrics.
• A demonstrated passion for teamwork and collaboration across functions within healthcare environments.
• Familiarity with innovative population health management models.
• Bilingual proficiency is a plus, aiding in communication with a diverse patient base.

Compensation and Benefits

The salary for this role ranges from USD 115,000 to 150,000 per year, with variations based on experience and qualifications. Benefits include:
