Work Environment

You will work onsite in an assisted living facility in North Tonawanda, offering a supportive and collaborative atmosphere where your contributions directly impact your patients' lives. The facility is designed to cater to the evolving needs of an aging population, ensuring they receive timely and effective healthcare services. Your working hours will typically span from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and you may also be involved in community outreach initiatives, including health fairs on weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents, creating individualized care plans that address both acute and chronic health issues.
• Interpret diagnostic tests and lab results to make informed clinical decisions while managing ongoing health conditions and promoting preventive care practices.
• Develop and implement educational programs aimed at improving health literacy and wellness among facility residents and their families.
• Provide culturally competent care, advocating for patients’ rights and ensuring that all patients have equitable access to necessary healthcare services.
• Coordinate with internal staff and external specialists for referrals and follow-up care to streamline patient pathways and foster continuity of care.
• Maintain precise and compliant medical records using electronic health record systems to ensure quality documentation and facilitate care coordination.
• Engage actively in health fairs and community outreach projects, emphasizing health screenings and preventive health measures in underserved populations.
• Stay informed on current public health trends, evidence-based practices, and changes in healthcare policy that affect practice within the community.

Patient Population / Client Focus

The primary focus of this role is the elderly population living in an assisted living facility. You will address a variety of healthcare needs, including chronic disease management, preventive care, mental health support, and transitions of care. Understanding the complexities of caring for older adults, you will work diligently to improve their quality of life and health outcomes.

Qualifications

• Current certification as a Nurse Practitioner.
• An active nursing license in the state of New York.
• A Bachelor’s degree in nursing or a related field from an accredited institution.
• A Master's degree in nursing, with specialized training as a Family Nurse Practitioner.
• A minimum of 3 years of clinical experience, particularly in community health or assisted living settings.
• Exceptional interpersonal and communication skills, with a demonstrated ability to work with diverse cultural and socioeconomic backgrounds.

Preferred Experience

Compensation and Benefits

Spectra Community Health offers a competitive annual salary ranging from $180,000 to $200,000, commensurate with your experience and qualifications.
