Bistro Walk-in Clinic is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our dynamic healthcare team in Tonawanda, New York. As a full-time FNP, you will play a critical role in delivering high-quality, holistic care to individuals and families in our community. Work Environment

This onsite role is based at our Home Care facility in Tonawanda. You will enjoy a supportive work environment defined by collaboration and teamwork. The clinic operates from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community outreach events, including weekend health fairs, may also be required. Your average workweek will be approximately 40 hours, enabling you to balance professional and personal commitments effectively.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, focusing on their unique healthcare requirements.
• Diagnose and manage various acute and chronic medical conditions, offering patient-centered preventive care and health promotion strategies.
• Work collaboratively with community organizations to design and implement impactful health education programs that cater to local needs.
• Provide compassionate and culturally sensitive care while advocating for patients' rights, ensuring they have access to necessary healthcare services.
• Coordinate patient referrals and ensure continuous follow-up care to promote better health outcomes.
• Maintain meticulous and accurate medical records in accordance with regulatory standards and best practices.
• Engage in community outreach to raise awareness about important health issues and provide screenings that can lead to early intervention.
• Stay abreast of community health trends, evidence-based practices, and relevant healthcare policies to enhance clinical practice.

Patient Population / Client Focus

The FNP will work primarily with underserved populations in the Tonawanda area, addressing diverse healthcare needs ranging from acute episodic conditions to chronic disease management. The clinic serves individuals and families from various backgrounds, emphasizing cultural competence and a commitment to health equity.

Qualifications

• Current certification as a Nurse Practitioner, with an active nursing license in the state of New York.
• A minimum educational requirement of a Master’s degree in nursing from an accredited program.

• Strong interpersonal and communication skills are essential for effectively engaging with diverse patient populations.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with experience and qualifications.
