Work Environment

You will practice in an onsite outpatient facility primarily focusing on family medicine. Our collaborative healthcare team includes physicians, nurse practitioners, physician assistants, and medical assistants, along with the support of dedicated medical scribes, registered nurses, and care managers. We maintain strong partnerships with top specialists and hospitals to ensure our patients have access to comprehensive services when needed. Key Responsibilities

• Provide exceptional primary care services, including routine and acute care visits, to ensure comprehensive patient management.
• Administer annual wellness visits and health risk assessments, focusing on holistic care and precise documentation.
• Coordinate care with a variety of healthcare professionals, specialists, and agencies to streamline patient services.
• Engage in population health leadership, ensuring eligible patients receive necessary screenings, such as biannual breast cancer checks for women.
• Assist with phone triage and outreach activities to follow up on patient care needs.
• Educate patients about their health conditions, care plans, and ongoing treatments, fostering a collaborative caregiving process.
• Participate in promotional activities to enhance community engagement and patient welfare.
• Conduct home visits as required to ensure excellent patient care.
• Report directly to the Medical Director while collaborating closely with operational leadership.

Patient Population / Client Focus

You will focus on providing personalized care to older adult patients on Medicare, a vital demographic that often requires tailored health management strategies. By creating trust and rapport with your patients, you’ll help them better understand their health conditions and become active participants in their care. Your work will directly contribute to their overall health outcomes and quality of life.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active and non-probationary Nurse Practitioner license in New York State.
• Current DEA license.
• Demonstrated experience in primary care settings, with a focus on internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A collaborative spirit with a passion for teamwork, ideally having experience in innovative population health strategies.
• Bilingual proficiency is a plus.

Preferred Experience

Ideal candidates will have prior experience working with Medicare populations and should exhibit strong capabilities in managing chronic health issues prevalent among older adults. Experience in home visits, care coordination, and using electronic health record systems will be highly valued.

Compensation and Benefits

This full-time Nurse Practitioner position offers a salary range of $115,000 to $150,000 annually, commensurate with experience and expertise. In addition, we provide a robust benefits package including:
