Work Environment

The position is based onsite at a local school, where you will interact closely with students, families, and educators. The work environment is dynamic, fostering direct engagement with patients as well as collaboration with multidisciplinary teams. You will work a standard schedule from 9:00 a.m. to 6:00 p.m., with some flexibility to accommodate evening clinics until 8:00 p.m. While an average week consists of about 40 hours, participation in weekend events such as health fairs and community outreach activities may be necessary to fulfill our commitment to the community.

Key Responsibilities

• Conduct thorough health assessments, including physical exams, immunization reviews, and screening procedures for students and families, ensuring comprehensive care delivery.
• Diagnose and manage both acute and chronic health conditions, putting an emphasis on preventive strategies and health promotion tailored to individual needs.
• Design and implement health education programs aimed at increasing awareness of common health issues such as nutrition, mental health, and preventive care.
• Advocate for patients’ rights and equitable access to healthcare services, actively addressing barriers that underserved populations face.
• Coordinate and manage referrals to specialists, ensuring that patients receive appropriate follow-up care and that transitions in care are seamless.
• Maintain accurate electronic medical records in accordance with regulatory requirements and organizational standards, facilitating quality assurance and patient safety.
• Engage in community outreach initiatives, providing health screenings and educational resources that empower individuals to take charge of their health.
• Stay informed about current trends in community health, emerging best practices, and relevant healthcare policies to ensure the highest quality of care.

Patient Population / Client Focus

Your work will primarily focus on serving the youth and families of Niagara Falls, an area marked by rich diversity but also by significant healthcare disparities. You will be a crucial ally in promoting health awareness, preventive care, and lifestyle management tailored to mitigate these disparities. By partnering with schools and community organizations, you will assess the unique healthcare needs of these populations, ensuring culturally sensitive care that acknowledges different backgrounds and needs.

Qualifications

• Active certification as a Family Nurse Practitioner from a nationally recognized accrediting body.
• A valid and active nursing license in New York State.
• A Master’s degree in nursing from an accredited institution.
• A minimum of three years of clinical experience, ideally within community health or public health settings.
• Exceptional interpersonal and communication skills that allow for effective collaboration with diverse populations.
• A strong commitment to social justice, health equity issues, and advocacy for vulnerable individuals and communities.

Preferred Experience

• Experience in school-based health programs or pediatric care.
• Familiarity with community health initiatives and outreach programs.
• Proficiency with electronic health record (EHR) systems to streamline patient management.
• Demonstrated ability to develop and implement health education curricula or community workshops.

Compensation and Benefits
