Work Environment

This role is based onsite at a local school, allowing you to engage primarily with children and their families in a familiar environment. Key Responsibilities

• Conduct thorough health assessments and physical examinations for patients of all ages, gathering comprehensive medical histories and understanding developmental milestones in children.
• Order, perform, and interpret essential diagnostic tests such as laboratories, X-rays, and other imaging studies, using results to guide clinical decision-making.
• Develop and implement individualized, patient-centered care plans in conjunction with a collaborative healthcare team, ensuring holistic approaches to treatment.
• Prescribe medications as appropriate while using evidence-based guidelines to determine effective treatment plans.
• Offer health education sessions to patients and their families, focusing on nutrition, exercise, preventive measures, and chronic disease management.
• Participate actively in interdisciplinary team meetings and case conferences to share insights on patient care and coordinate services efficiently.
• Maintain accurate, detailed medical records using electronic health record (EHR) systems, documenting patient encounters following best practices.
• Engage in continuous quality improvement initiatives to help enhance patient outcomes and overall care delivery.
• Advocate for school health programs and collaborate on initiatives aiming to promote student wellness.

Patient Population / Client Focus

In this role, you will primarily serve a pediatric population, from newborns to adolescents. Your commitment to preventive care and health education will be essential in promoting healthy growth and development among young patients. You will also address a variety of common health concerns such as allergies, asthma, mental health issues, and obesity, ensuring thorough follow-up and support.

Qualifications

• A valid and unrestricted Nurse Practitioner license in New York State.
• A Bachelor’s Degree in Nursing or a related field; a master’s degree is preferred and may enhance your candidacy.
• Strong clinical assessment and diagnostic skills, demonstrated through active patient care roles.
• Proficiency with EHR systems, particularly in documenting patient care and managing health information accurately.
• Exceptional communication skills to effectively educate and counsel patients and families on health-related topics.

Preferred Experience
