In this essential role, you will collaborate closely with a multidisciplinary healthcare team, managing patients in an outpatient setting while focusing on pain management and orthopedic care under the guidance of our Chief Nurse Practitioner and Medical Director.

Work Environment

This Nurse Practitioner position operates onsite at an assisted living facility, ensuring a dynamic and supportive work environment. You will engage with patients, fellow nurse practitioners, physicians, and support staff on a daily basis, fostering a culture of teamwork and shared responsibility. Key Responsibilities

• Conduct comprehensive patient assessments, taking thorough histories and performing physical examinations.
• Develop and implement individualized care plans, providing outpatient services for conditions such as hypertension, high cholesterol, and other chronic ailments.
• Deliver specialized care in Electrophysiology, including managing patients with arrhythmias and device patients.
• Facilitate stress testing and interpret results to guide clinical decisions.
• Manage acute conditions such as Coronary Artery Disease, Peripheral Artery Disease, Congestive Heart Failure, and Transient Ischemic Attacks.
• Educate patients and their families regarding treatment options, self-care strategies, and lifestyle modifications to support better health outcomes.
• Coordinate referrals to specialists within our orthopedic team as needed, ensuring continuity of care.
• Participate in quality improvement initiatives and ongoing professional development opportunities.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse outpatient population with varying orthopedic needs. Patients range from those seeking pain management for chronic conditions to individuals needing pre-operative evaluations for orthopedic procedures. Qualifications

• Successful completion of an accredited Nurse Practitioner program with a focus on Orthopedics or related fields.
• Possession of a Master's or doctoral degree in Nursing.
• Current licensure as a Nurse Practitioner in the state of New York.

Compensation and Benefits

The compensation for this position is competitive, with an annual salary range of USD 120,000 – 160,000, commensurate with experience and qualifications. In addition to a competitive salary, our benefits package includes:
